Consumer Rights

Texas consumers have the right to:

• Request access to their health care records from this practice.
• Contact the Texas Behavioral Health Executive Council regarding psychological services provided in Texas.
• File a consumer complaint with the Office of the Texas Attorney General.

Requesting Your Health Care Records

You may request access to health care records maintained by Common Ground Therapy & Wellness PLLC by submitting a written request through the practice’s secure contact page:

Please include your name, contact information, and a brief statement that you are requesting access to your health care records. Do not include sensitive clinical information in an unsecured website message.

Consumers may contact the Texas Behavioral Health Executive Council regarding the regulation of psychological services in Texas or to obtain information about filing a professional complaint.

Authorization to Provide Telepsychology Services

Kelsey Ellis, PhD, is a psychologist licensed in Virginia and is authorized to provide telepsychology services to eligible clients located in Texas through the Psychology Interjurisdictional Compact, also known as PSYPACT.
